Annual Discipline-Specific Professional Development Documentation and Verification 

Purpose

This form documents annual discipline-specific professional development and professional collaboration between the HTC Faculty Liaison and High School Instructor.

The completed documentation provides evidence that High School instructors participate in discipline-specific professional development and collaborate with HTC faculty to support course alignment, instructional practices, and continuous improvement.

Review Process and Instructions

1. One completed form is required annually for each Faculty Liaison–High School Instructor pairing and each Concurrent Enrollment discipline area.
2. The Faculty Liaison and High School Instructor will collaborate to identify discipline-specific professional development that supports continuous improvement of the Concurrent Enrollment course.
3. The selected professional development activity should support continued discipline-specific knowledge and professional growth, as well as course alignment, instructional practices, assessment practices, student learning, or other course-related improvement efforts.
4. Following completion of the professional development activity, the Faculty Liaison and High School Instructor will document key takeaways, professional collaboration, and application to the Concurrent Enrollment course.
5. The completed form and required supporting evidence must be submitted by the stated deadline.
